The following is a list of the paperwork requirements to complete your personnel file:

1. Copy of your NYS current registration;
   
2. Copy of NYS original license;
   
3. Copy of diploma - proof of degree;
   
4. Proof of current liability insurance;
   
5. Physical exam to include:
        · Mantoux test for TB - date and result (chest x-ray if TB test is positive)
        · Rubella titre - date and result
        · Rubeola titre - date and result
        · Varicella - date and result
   
6. Current resume;
7. Two (2) recent professional letters of reference;
8. Copy of social security card;
   
9. Proof of 2 inservice trainings within the last year;
10. Proof of Workmen's Compensation Insurance. (In order to obtain workmens compensation coverage you may contact either the NY State Insurance Fund at www.nysif.com;
